The Power of Planning Well: Advance Serious Illness Planning

In this webinar hosted by Family Caregivers of BC, Dr. Daren Heyland, founder of Plan Well Guide and critical care physician and researcher, will present on the Plan Well Guide, an aid to support decision-making in serious illness.

The focus of the webinar is to highlight the current deficiencies in communication, decision-making and planning process related to serious illness. You should leave this presentation knowing what Plan Well Guide is and how it can assist you with specific planning issues.

About Dr. Daren Heyland

Dr Daren Heyland is a critical care doctor, a medical researcher, and a Professor of Medicine at Queen’s University, Kingston, Ontario Canada where he serves as the Director of the Clinical Evaluation Research Unit at the Kingston General Hospital.  He is a champion of person-centered care and has over his career developed a number of tools and strategies to enable lay people to understand serious illness decision-making so they get the medical care that is right for them (or their loved ones). He has developed and maintains free online decision aids at www.planwellguide.com.

Plan Well Guide

Plan Well Guide is a free, advance serious illness planning tool. The Plan Well Guide can help you learn how serious illness decisions are made. You’ll learn about how important your values and preferences are in making those decisions. This guide can help you make a plan that clearly let’s others, like your doctor, family, and substitute decision-maker, know what you want for your care. By doing your serious illness planning ahead of time, you’re more likely to get the medical care that’s right for you. It can also lower the stress and anxiety your family member’s will have if they need to make life or death decisions for you.
